Effective date: July 20, 2026
1. Scope
This Website Privacy Policy explains how Jon M. Grazer, MD, MPH, FACS (the “Practice,” “we,” “us,” or “our”) collects, uses, discloses, and protects information obtained through www.drgrazer.com and related online forms, landing pages, and digital communications.
This policy applies to website visitors and prospective patients. It does not replace our Notice of Privacy Practices, which governs protected health information maintained by the medical practice under applicable health privacy laws.
2. Information We Collect
We may collect information that you provide directly, including your name, telephone number, email address, procedure or service interests, appointment preferences, and the content of messages submitted through the website.
We may automatically collect technical and usage information, including IP address, browser type, device information, referring page, pages viewed, approximate location derived from IP address, date and time of access, and interactions with website features.
We may use cookies, pixels, analytics tools, call-tracking tools, and similar technologies. These technologies may be operated by service providers that help us host, secure, analyze, advertise, and improve the website.
3. How We Use Information
We may use information to respond to inquiries, schedule consultations, provide requested information, communicate about appointments or services, operate and secure the website, maintain records, improve our services, measure advertising performance, comply with law, and protect the rights and safety of the Practice, our patients, and others.
We do not use an online inquiry as a substitute for an in-person medical evaluation. Do not use the website or text messaging for emergencies. Call 911 for a medical emergency.
4. Text Messaging and Mobile Information
If you separately consent to receive text messages, we may use your mobile number to send consultation follow-up, appointment-related, service-related, and other messages described at the point of consent.
Message frequency varies. Message and data rates may apply. Reply STOP to cancel and HELP for help.
Mobile opt-in data, mobile numbers, and text messaging consent will not be sold, rented, or shared with third parties, affiliates, or lead generators for their marketing or promotional purposes. We may disclose this information to service providers that process messages solely on our behalf and are contractually restricted from using it for their own purposes, except as necessary to provide services to the Practice.
This text messaging program is also governed by our SMS Terms and Conditions.
5. When We Disclose Information
We may disclose information to vendors and service providers that support website hosting, information technology, analytics, advertising, call handling, scheduling, customer relationship management, text messaging, email delivery, security, payment processing, and professional services.
We may also disclose information when required by law; to respond to legal process; to protect rights, safety, or property; in connection with a business reorganization or transfer; or with your direction or consent.
We do not sell personal information for money. We may use analytics and advertising technologies operated by service providers to help us understand website use and measure advertising performance. Some of these disclosures may be treated as a “sale” or “sharing” under the California Consumer Privacy Act or other state privacy laws. Where those laws apply to the Practice, we will provide an appropriate method to exercise applicable opt-out rights.
6. Healthcare and Sensitive Information
A general website inquiry form is not a patient portal and should not be used to transmit Social Security numbers, payment-card information, detailed medical records, photographs, or urgent medical concerns. Use the Practice’s designated secure communication method when requested.
Once information is received and maintained as part of the Practice’s medical or billing records, different privacy rules may apply, including our Notice of Privacy Practices.
7. Cookies and Analytics
We may use cookies and similar technologies to operate the website, maintain security, remember preferences, understand website traffic, evaluate marketing, and improve performance. Browser settings may allow you to block or delete cookies, but some website functions may not work properly. We assess the third-party analytics and advertising technologies we use and configure them to limit the collection and disclosure of sensitive information. This Privacy Policy is not, by itself, authorization to disclose protected health information to an analytics or advertising provider.
8. Data Retention and Security
We retain information for as long as reasonably necessary for the purposes described in this policy, to maintain business and medical records, resolve disputes, enforce agreements, and comply with legal obligations.
We use reasonable administrative, technical, and physical safeguards. No website, email, text message, or electronic storage system is completely secure, and we cannot guarantee absolute security.
9. Your Choices and Rights
You may ask us to update or correct contact information, stop marketing communications, or address a privacy question by contacting us using the information below. Text-message opt-out requests may be submitted by replying STOP.
Depending on the law that applies to us and your relationship with the Practice, you may have rights concerning certain personal information we maintain, which may include rights to request access, correction, deletion, or restriction of certain uses or disclosures. These rights are subject to legal exceptions and may not apply to information governed by medical-record or healthcare-privacy laws. To submit a privacy request, contact us using the information in Section 13. We may need to verify your identity before processing a request.
10. Children
The website is intended for adults. We do not knowingly collect personal information from children under 13 through the website without appropriate authorization.
11. External Links
The website may link to third-party sites or services. Their privacy practices are governed by their own policies, not this policy.

We may revise this policy from time to time. The updated version will be posted on this page with a new effective date.
