SMS Terms and Conditions

1. Program Description

By opting in, you authorize Jon M. Grazer, MD, MPH, FACS (the “Practice,” “we,” “us,” or “our”) to send SMS and MMS messages to the mobile number you provide. Messages may include responses to your inquiry, consultation follow-up, appointment scheduling and reminders, pre-appointment instructions, office updates, and information about services you requested.

We send promotional or marketing messages only when the consent language presented to you at opt-in expressly covers them.

2. Consent

Consent to receive text messages is not a condition of purchasing any goods or services. You may continue to contact the office by telephone or email if you do not wish to receive texts.

You represent that you are the subscriber or customary user of the mobile number provided and that you are authorized to consent to messages at that number.

3. Message Frequency and Charges

Message frequency varies based on your interactions with the office. Message and data rates may apply. Your mobile carrier’s rates and terms govern charges.

4. Opt-Out

Reply STOP to any message to cancel. You may receive one final message confirming the opt-out. After opting out, you will not receive additional messages from that messaging program unless you provide new consent.

Other commonly supported opt-out words may include CANCEL, END, QUIT, STOPALL, and UNSUBSCRIBE, depending on the messaging platform.

6. Delivery

Wireless carriers are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission by your carrier and is not guaranteed.

7. Privacy

Your use of the text messaging program is also governed by the Website Privacy Policy.

Mobile opt-in data and consent will not be sold or shared with third parties, affiliates, or lead generators for their marketing or promotional purposes. Information may be processed by messaging and technology service providers solely to operate the program on our behalf.

8. No Emergency or Medical Advice

Text messaging is not monitored continuously and must not be used for emergencies, urgent medical concerns, or time-sensitive postoperative issues. Call 911 for a medical emergency. Use the office’s designated urgent-contact procedure for urgent clinical concerns.

Text messages do not create a physician-patient relationship and are not a substitute for medical advice, diagnosis, or treatment.

9. Supported Carriers and Number Changes

Participating carriers and service availability may change. If you change, deactivate, or transfer your mobile number, notify the office and opt out before the change when possible.

10. Changes to These Terms

We may update these terms. Updated terms will be posted on the website with a revised effective date. Material changes to a messaging program may require renewed consent.
